Pantop 40 from Aristo Pharmaceuticals is Pantoprazole Gastro-Resistant Tablets IP 40 mg, made from pantoprazole sodium. Among the proton pump inhibitors, pantoprazole is the one chosen when a patient is on several other medicines at once. All PPIs are handled by the liver’s cytochrome P450 system, but pantoprazole has the lowest potential for interaction of the group – after its first pass it is finished off by a separate enzyme system, so it interferes far less with warfarin, phenytoin, clopidogrel, digoxin and the rest. For an older patient on a long medicine list, that is often the deciding factor rather than potency. It is also the most pH-stable of the PPIs, which is why the intravenous form is so widely used in hospitals. The tablet is gastro-resistant and coloured with Tartrazine and Titanium Dioxide IP. Swallow whole. 15 tablets.

Directions for Use

  • Take it 30 to 60 minutes before a meal, usually before breakfast, or exactly as your doctor directs
  • Swallow it whole with water. Do not chew, crush or break it – the enteric coating is what gets the drug past stomach acid intact
  • Take it at the same time each day, and finish the full course even if the symptoms settle early
  • If you miss a dose, take it when you remember unless the next is nearly due. Never take two together
  • Avoid or limit alcohol, smoking, and food that triggers your symptoms while you are being treated

Precautions

  • Take it 30 to 60 minutes before food, not after. A proton pump inhibitor only shuts down pumps that are actively working, and they switch on when you eat – taken after a meal it does a fraction of the job
  • These are not antacids and do not work in minutes. Full acid suppression builds over three to five days; keep an antacid for immediate relief if your doctor agrees
  • Long-term use is not harmless. Months or years of treatment are linked to low magnesium, low vitamin B12, reduced calcium absorption and a higher risk of fractures, and to gut infections such as C. difficile. Use the lowest dose for the shortest time that controls symptoms
  • Do not stop a long course abruptly – acid can rebound above where it started, and the symptoms can be mistaken for the original problem returning. Ask your doctor about tapering
  • Tell your doctor about difficulty swallowing, vomiting blood, black stools, unintended weight loss or persistent vomiting. Acid suppression can mask a more serious problem
  • Tell your doctor about liver disease, lupus, low magnesium, and every other medicine you take – especially clopidogrel, methotrexate, digoxin, antifungals, HIV medicines and warfarin

Storage

  • Store protected from moisture, at a temperature not exceeding 30°C, as printed on the blister
  • Not to be chewed or crushed – to be swallowed whole
  • Keep out of reach of children
