Omez-40 from Dr Reddy’s is an Omeprazole Magnesium tablet equivalent to 40 mg of omeprazole – and the magnesium salt in the name is not a detail to skip over. Plain omeprazole is chemically fragile and breaks down quickly in an acidic or damp environment, which is why almost every omeprazole product is a capsule of enteric-coated pellets. Forming the magnesium salt makes the molecule stable enough to be compressed into a tablet at all, and gives it better solubility and a slightly more consistent absorption profile. The 40 mg is the high-dose end of the range: it is used where 20 mg has not been enough – severe erosive oesophagitis, ulcers that keep recurring, and Zollinger-Ellison syndrome – and it is a dose that should have a defined end point rather than being taken indefinitely. Directions for Use

  • Take it 30 to 60 minutes before a meal, usually before breakfast, or exactly as your doctor directs
  • Swallow it whole with water. Do not chew, crush or break it – the enteric coating is what gets the drug past stomach acid intact
  • Take it at the same time each day, and finish the full course even if the symptoms settle early
  • If you miss a dose, take it when you remember unless the next is nearly due. Never take two together
  • Avoid or limit alcohol, smoking, and food that triggers your symptoms while you are being treated

Precautions

  • Take it 30 to 60 minutes before food, not after. A proton pump inhibitor only shuts down pumps that are actively working, and they switch on when you eat – taken after a meal it does a fraction of the job
  • These are not antacids and do not work in minutes. Full acid suppression builds over three to five days; keep an antacid for immediate relief if your doctor agrees
  • Long-term use is not harmless. Months or years of treatment are linked to low magnesium, low vitamin B12, reduced calcium absorption and a higher risk of fractures, and to gut infections such as C. difficile. Use the lowest dose for the shortest time that controls symptoms
  • Do not stop a long course abruptly – acid can rebound above where it started, and the symptoms can be mistaken for the original problem returning. Ask your doctor about tapering
  • Tell your doctor about difficulty swallowing, vomiting blood, black stools, unintended weight loss or persistent vomiting. Acid suppression can mask a more serious problem
  • Tell your doctor about liver disease, lupus, low magnesium, and every other medicine you take – especially clopidogrel, methotrexate, digoxin, antifungals, HIV medicines and warfarin

Storage

  • Store below 30°C in a dry place, protected from light
  • Keep the tablets in the blister until use
  • Keep out of reach of children
