Tsart Trio 12.5 Tablet – Telmisartan 40 mg + Cilnidipine 10 mg + Chlorthalidone 12.5 mg by Alkem

Tsart Trio 12.5 from Alkem’s Metabolics division is a film-coated tablet of Telmisartan IP 40 mg, Cilnidipine 10 mg and Chlorthalidone IP 12.5 mg. The component that most often surprises people is chlorthalidone. It looks like an ordinary thiazide diuretic and is usually swapped in at the same 12.5 mg mark, but it is roughly twice as potent as hydrochlorothiazide milligram for milligram and stays active for two to three days rather than half of one. That long tail is why it keeps night-time and early-morning readings down, the window in which most strokes and heart attacks occur, and why the large outcome trials that changed hypertension guidelines used chlorthalidone rather than its shorter cousin. Paired with telmisartan – whose own effect outlasts 24 hours – and with cilnidipine for arterial tone, this tablet is built around the idea that the whole day matters, not just the hours after a dose. Directions for Use

• Swallow the tablet whole with water. Do not crush or chew it, as printed on the pack
• Take it once a day at the same time each morning – the diuretic works better early than at bedtime
• It can be taken with or without food, but keep the routine consistent
• Take it exactly as your doctor directs. The dose is decided by your readings, not by how you feel
• Do not stop on your own when your readings improve – that is the medicine working, not the problem going away

Precautions

• Do not take this if you are pregnant or planning a pregnancy. The pack carries a warning that it must be stopped as soon as pregnancy is discovered – telmisartan harms the developing baby in the second and third trimesters
• Your doctor will check potassium, sodium, uric acid, blood sugar and kidney function periodically. Chlorthalidone can lower potassium and sodium while raising uric acid and sugar; telmisartan pushes potassium the other way
• Tell your doctor if you have gout, diabetes, kidney or liver disease, or a low blood volume from vomiting or diarrhoea
• Avoid potassium supplements and salt substitutes unless your doctor approves them. Aliskiren is not to be combined with an ARB in diabetes
• Dizziness on standing is most likely with the first dose – get up slowly, and do not drive until you know how it affects you
• Report a persistent dry cough, swelling of the face or lips, unusual cramps or a very low urine output straight away

Storage

• Store in a cool and dry place. Protect from light, as printed on the pack
• Leave the tablets sealed in the blister until use
• Keep out of reach of children
