Topinate Cream from Systopic Laboratories is Clobetasol Propionate IP 0.05% w/w in a 30 g tube. Clobetasol sits at the very top of the topical steroid ladder – it is a super-potent, class I corticosteroid, roughly six hundred times the strength of hydrocortisone, and that single fact governs how it should be used. It is prescribed for thick, stubborn, steroid-resistant plaques: severe psoriasis, lichen planus, discoid lupus, chronic hand eczema – the conditions where a mild steroid simply bounces off. It is not a general-purpose itch cream. Dermatology practice keeps it to short courses, usually no more than two to four weeks, with a limit on how much is used per week, and keeps it off the face, the groin and the armpits, where the skin is thin and absorbs far more. Used that way it is highly effective; used casually and indefinitely it thins the skin permanently. 30 g tube, for external use only.

Uses & Benefits

Directions for Use

  • For external use only. Wash and dry the affected area, then apply a thin film – more does not work better
  • Wash your hands after applying, unless the hands are the area being treated
  • Keep it away from the eyes, nose and mouth. Rinse with plenty of water if it gets there by accident
  • Do not cover the treated area with an airtight dressing or bandage unless your doctor tells you to – that multiplies absorption
  • Usually twice daily, or exactly as prescribed. Do not use it for more than four consecutive weeks at a time

Precautions

  • This is a super-potent steroid. Do not use it on the face, groin or underarms unless your doctor specifically directs it – the skin there is thin and absorbs far more
  • Prolonged use thins the skin permanently and can cause stretch marks, visible veins, easy bruising and acne-like eruptions. Long or large-area use can also suppress the adrenal glands
  • Do not use it on an infected patch of skin. Steroids can let fungal, bacterial and viral skin infections spread unchecked
  • Stop and see your doctor if the treated area becomes infected, or if there is no improvement after four weeks
  • Not recommended in children, whose skin absorbs more relative to body size
  • Pregnant and breastfeeding women should consult their doctor before using it. Burning, stinging, irritation or itching at the site is the usual early effect

Storage

  • Store at a temperature not exceeding 30°C. Do not freeze, as printed on the tube
  • Puncture the nozzle seal with the piercing point of the cap; keep the cap on
  • Keep the medicine out of the reach of children
