HISone-20 from Samarth Life Sciences is an uncoated tablet of Hydrocortisone IP 20 mg. Hydrocortisone is different from every other oral steroid on the shelf, because it is not really a substitute for anything – it is identical to cortisol, the hormone the adrenal glands make themselves. Prednisolone and dexamethasone are synthetic analogues chosen for potency and long action; hydrocortisone is chosen for the opposite reason. Its short duration lets a dose given in the morning and a smaller one at midday imitate the body’s own daily cortisol rhythm, which is why it is the standard replacement therapy in adrenal insufficiency and Addison’s disease, and why 20 mg is such a common strength – it is close to the daily output of a healthy adrenal gland. It is also used at anti-inflammatory doses for allergic and inflammatory skin disorders such as eczema, atopic dermatitis and psoriasis. Never stop it suddenly. Strip of 10 tablets.

Directions for Use

  • Swallow the tablet whole with water. Do not chew, crush or break it
  • Take it with or just after food – this prevents stomach upset
  • Take it regularly and at the times your doctor sets. In replacement therapy the timing of each dose matters as much as the total
  • If you miss a dose, take it as soon as you remember unless the next is nearly due. Never double up
  • Do not stop taking it on your own. The dose has to be reduced gradually

Precautions

  • Never stop an oral corticosteroid abruptly. Stopping suddenly after more than a short course can cause a dangerous adrenal crisis – the dose must be tapered by your doctor
  • If you take it as adrenal replacement, the dose usually needs increasing during illness, fever, injury or surgery. Ask your doctor for a sick-day rule and carry a steroid card
  • It makes infections harder to fight and can mask them. Tell your doctor about any fever, sore throat or other sign of infection, and avoid people with chickenpox or measles if you have not had them
  • Long-term use can raise blood sugar and blood pressure, thin the bones, cause weight gain and mood changes, and affect the eyes
  • Tell your doctor about diabetes, high blood pressure, stomach ulcers, glaucoma, tuberculosis, osteoporosis or any mental health condition, and about every other medicine you take
  • Dizziness, headache, swollen ankles and weakness can occur. Inform your doctor if you are pregnant or breastfeeding

Storage

  • Store at a temperature not exceeding 25°C. Protect from light and moisture, as printed on the blister
  • Keep the tablets in the blister until use
  • Keep out of reach of children
