Cilnep-Trio from Leeford Healthcare puts Telmisartan 40 mg, Cilnidipine 10 mg and Chlorthalidone 12.5 mg into one tablet. A three-drug tablet exists because high blood pressure that resists treatment usually resists it for more than one reason, and the three ingredients attack three separate ones: telmisartan blocks the hormone signal that constricts vessels, cilnidipine blocks the calcium channels that keep vessel muscle contracted, and chlorthalidone reduces the fluid volume the heart has to move. Chlorthalidone is the diuretic worth noting – it is longer-acting than hydrochlorothiazide and, in outcome trials, has performed better at preventing cardiovascular events. Directions for Use

  • Take one tablet once daily, exactly as prescribed, at the same time each day
  • Take it in the morning – the diuretic increases urination, and an evening dose disturbs sleep
  • Swallow the tablet whole with water; may be taken with or without food
  • Keep taking it even when you feel well – high blood pressure usually has no symptoms
  • Do not stop or change the dose on your own; pressure rebounds when treatment stops

Precautions

  • Stop immediately and contact your doctor if you become pregnant. The pack warns that telmisartan can cause injury and even death to the developing baby when used in the second and third trimesters
  • Seek emergency care for swelling of the face, lips, tongue or throat
  • The diuretic can lower potassium and sodium and raise uric acid and blood sugar. Tell your doctor if you have gout or diabetes, and keep to the blood test schedule
  • Dizziness on standing is common early on, and worse if you are dehydrated
  • Tell your doctor about kidney disease, renal artery narrowing, liver disease, or if you take NSAIDs or lithium
  • Do not combine with an ACE inhibitor or aliskiren unless a specialist directs it
