Sompraz IV from Sun Pharmaceutical Industries is Esomeprazole Sodium for Injection, 40 mg per vial, lyophilized, supplied as a combi pack with one 5 ml vial of Sodium Chloride Injection IP 0.9%. It is for intravenous administration only and is used where the oral route is unavailable or too slow – a patient who is vomiting, nil by mouth, unconscious, or bleeding from an upper gastrointestinal ulcer. The reason it is a freeze-dried powder rather than a ready solution is chemical: esomeprazole in solution degrades within hours, and the colour of the solution changes as it does. Freeze-drying keeps it stable on the shelf and puts the clock on only after reconstitution, which is why it must be prepared immediately before use, diluted before administration, and any unused portion discarded. Composition

• Esomeprazole Sodium – equivalent to Esomeprazole 40 mg per vial. Excipients q.s.

Uses & Benefits

• Controls acid where a patient cannot take medicine by mouth
• Used in upper gastrointestinal bleeding from a peptic ulcer, as prescribed
• Used for severe reflux oesophagitis and ulcer disease in hospital

Directions for Use

• This injection is given by a doctor or nurse. It is not for self-administration
• Reconstitute with the supplied sodium chloride injection and dilute before intravenous administration, following the prescribing information
• Prepare it immediately before use and use it promptly – esomeprazole degrades in solution
• Do not use if any foreign particulate matter is visible in the vial, or if the solution has discoloured
• Single use only. Discard any unused portion

Precautions

• For intravenous use only. It is not to be given intramuscularly or by any other route
• Intravenous treatment is a short-term measure – patients are switched to an oral PPI as soon as they can swallow
• Tell the treating doctor about liver disease, low magnesium, and every other medicine, especially clopidogrel, methotrexate, digoxin, antifungals and HIV medicines
• Prolonged acid suppression carries the same concerns as the oral form – low magnesium, low vitamin B12 and a higher risk of gut infection
• Tell the doctor about any previous reaction to esomeprazole, omeprazole or another PPI
• Tell the doctor if the patient is pregnant or breastfeeding

Storage

• Store at a temperature not exceeding 25°C. Protect from light, as printed on the carton
• Keep the vial in its carton until use
• Keep out of reach of children
