Rabium-DSR Capsule – Rabeprazole Sodium 20 mg + Domperidone SR 30 mg by Intas

Rabium-DSR from Intas Pharmaceuticals is a hard gelatin capsule containing Rabeprazole Sodium IP 20 mg as enteric-coated pellets and Domperidone IP 30 mg as sustained-release pellets. The reason this is a capsule rather than a tablet is worth understanding, because it explains why it must never be opened. The two drugs need opposite handling: rabeprazole is destroyed by stomach acid and has to be enteric-coated so it passes through the stomach untouched, while domperidone works in the gut wall and is formulated for slow release across the day. Two incompatible release profiles cannot be pressed into one tablet, so each drug is made into its own pellets and the two are filled into a shell that simply holds them together. Crush or open it and you destroy the rabeprazole and dump the whole domperidone dose at once. Domperidone is not a drug for indefinite use – it prolongs the QT interval and belongs in short courses. Directions for Use

  • Swallow the capsule whole with water. Do not open, chew or crush it – it contains two different pellet types with two different release profiles
  • Take it 30 to 60 minutes before a meal, usually before breakfast, or exactly as your doctor directs
  • Take it at the same time each day, for the period your doctor sets
  • If you miss a dose, take it when you remember unless the next is nearly due. Never double up
  • This combination is meant for short courses. Ask your doctor before continuing it beyond the prescribed period

Precautions

  • This combination contains domperidone. Domperidone can affect heart rhythm by prolonging the QT interval, so it should be used at the lowest effective dose for the shortest possible time – usually no more than a week without review
  • Tell your doctor about any heart condition, heart rhythm problem, or low potassium or magnesium, and about medicines that also prolong QT such as certain antibiotics, antifungals and antidepressants
  • Do not use it if you have a gastrointestinal bleed, blockage or perforation – speeding the gut up in those situations is dangerous
  • Take the capsule 30 to 60 minutes before food and swallow it whole; never open or crush it
  • Tell your doctor about liver disease, kidney disease, or if you are pregnant or breastfeeding. Domperidone can raise prolactin and may cause breast tenderness or discharge
  • Do not stop a long course of the PPI abruptly, and tell your doctor about difficulty swallowing, vomiting blood, black stools or unintended weight loss

Storage

  • Store in a cool and dry place. Protect from light, as printed on the carton
  • Keep the capsules in the blister until use
  • Keep out of reach of children
