It is not simply “half a dose”: it is the maintenance and step-down strength, prescribed once the initial 20 or 40 mg course has healed the oesophagus or ulcer and the aim shifts to keeping symptoms away with the smallest effective dose. Directions for Use

• Take it 30 to 60 minutes before a meal, usually before breakfast, or exactly as your doctor directs
• Swallow it whole with water. Do not chew, crush or break it – the enteric coating is what gets the drug past stomach acid intact
• Take it at the same time each day, and finish the full course even if the symptoms settle early
• If you miss a dose, take it when you remember unless the next is nearly due. Never take two together
• Avoid or limit alcohol, smoking, and food that triggers your symptoms while you are being treated

Precautions

• Take it 30 to 60 minutes before food, not after. A proton pump inhibitor only shuts down pumps that are actively working, and they switch on when you eat – taken after a meal it does a fraction of the job
• These are not antacids and do not work in minutes. Full acid suppression builds over three to five days; keep an antacid for immediate relief if your doctor agrees
• Long-term use is not harmless. Months or years of treatment are linked to low magnesium, low vitamin B12, reduced calcium absorption and a higher risk of fractures, and to gut infections such as C. difficile. Use the lowest dose for the shortest time that controls symptoms
• Do not stop a long course abruptly – acid can rebound above where it started, and the symptoms can be mistaken for the original problem returning. Ask your doctor about tapering
• Tell your doctor about difficulty swallowing, vomiting blood, black stools, unintended weight loss or persistent vomiting. Acid suppression can mask a more serious problem
• Tell your doctor about liver disease, lupus, low magnesium, and every other medicine you take – especially clopidogrel, methotrexate, digoxin, antifungals, HIV medicines and warfarin

Storage

• Store below 25°C. Protect from light and moisture, as printed on the blister
• Keep the capsules in the blister until use
