Cilacar TC 12.5 from J. B. Chemicals & Pharmaceuticals is a film-coated tablet of Cilnidipine 10 mg, Telmisartan IP 40 mg and Chlorthalidone IP 12.5 mg. Cilacar is the brand that introduced cilnidipine widely in India, and the TC extension adds the two partners that make a stubborn reading move. What a triple combination buys you is not simply more force – it is fewer side effects for the same fall in pressure. Doubling a single drug tends to double its characteristic problem: more amlodipine means more swollen ankles, more diuretic means more potassium loss, more ARB alone means the body simply retains salt to compensate. Three low doses acting on three separate systems produce a larger drop than any one of them at full dose, while each stays at a level the body tolerates easily. Directions for Use

• Swallow the tablet whole with water. Do not crush or chew it, as printed on the pack
• Take it once a day at the same time each morning – the diuretic works better early than at bedtime
• It can be taken with or without food, but keep the routine consistent
• Take it exactly as your doctor directs. The dose is decided by your readings, not by how you feel
• Do not stop on your own when your readings improve – that is the medicine working, not the problem going away

Precautions

• Do not take this if you are pregnant or planning a pregnancy. The pack carries a warning that it must be stopped as soon as pregnancy is discovered – telmisartan harms the developing baby in the second and third trimesters
• Your doctor will check potassium, sodium, uric acid, blood sugar and kidney function periodically. Chlorthalidone can lower potassium and sodium while raising uric acid and sugar; telmisartan pushes potassium the other way
• Tell your doctor if you have gout, diabetes, kidney or liver disease, or a low blood volume from vomiting or diarrhoea
• Avoid potassium supplements and salt substitutes unless your doctor approves them. Aliskiren is not to be combined with an ARB in diabetes
• Dizziness on standing is most likely with the first dose – get up slowly, and do not drive until you know how it affects you
• Report a persistent dry cough, swelling of the face or lips, unusual cramps or a very low urine output straight away

Storage

• Store protected from light and moisture at a temperature not exceeding 30°C, as printed on the strip
• Tablets are to be swallowed whole, not crushed or chewed
• Keep out of reach of children
