Macsart CC 12.5 Tablet – Telmisartan + Cilnidipine + Chlorthalidone by Macleods

Macsart CC 12.5 from Macleods combines Telmisartan 40 mg, Cilnidipine 10 mg and Chlorthalidone 12.5 mg in one daily tablet. The cilnidipine in it is the part worth knowing about. Ordinary calcium channel blockers such as amlodipine act on L-type channels in the artery wall; they widen the arteries efficiently, but they also widen the inflow side of the capillary bed more than the outflow side, and fluid is pushed into the tissue – which is exactly why so many people on amlodipine end up with swollen ankles by evening. Cilnidipine additionally blocks N-type channels on sympathetic nerve endings, which dampens the noradrenaline surge, balances the two sides of the capillary bed, and suppresses both the ankle swelling and the reflex fast pulse. Add telmisartan for the hormonal arm and chlorthalidone for the fluid arm, and a resistant reading has few places left to hide. Strip of 10 tablets.

Directions for Use

• Swallow the tablet whole with water. Do not crush or chew it, as printed on the pack
• Take it once a day at the same time each morning – the diuretic works better early than at bedtime
• It can be taken with or without food, but keep the routine consistent
• Take it exactly as your doctor directs. The dose is decided by your readings, not by how you feel
• Do not stop on your own when your readings improve – that is the medicine working, not the problem going away

Precautions

• Do not take this if you are pregnant or planning a pregnancy. The pack carries a warning that it must be stopped as soon as pregnancy is discovered – telmisartan harms the developing baby in the second and third trimesters
• Your doctor will check potassium, sodium, uric acid, blood sugar and kidney function periodically. Chlorthalidone can lower potassium and sodium while raising uric acid and sugar; telmisartan pushes potassium the other way
• Tell your doctor if you have gout, diabetes, kidney or liver disease, or a low blood volume from vomiting or diarrhoea
• Avoid potassium supplements and salt substitutes unless your doctor approves them. Aliskiren is not to be combined with an ARB in diabetes
• Dizziness on standing is most likely with the first dose – get up slowly, and do not drive until you know how it affects you
• Report a persistent dry cough, swelling of the face or lips, unusual cramps or a very low urine output straight away

Storage

• Store in a cool, dry place below 30°C, protected from light and moisture
• Keep the tablets in the blister until you take them
• Keep out of reach of children
