Dilnip-Trio Tablet – Cilnidipine 10 mg + Telmisartan 40 mg + Chlorthalidone 12.5 mg by Lupin
Dilnip-Trio from Lupin brings together Cilnidipine 10 mg, Telmisartan IP 40 mg and Chlorthalidone IP 12.5 mg in a single daily tablet for blood pressure that has not settled on one or two medicines. The logic behind a triple pill is worth understanding: blood pressure is held up by three separate systems – the tone of the arteries, the renin-angiotensin hormone axis, and the volume of salt and water the body holds on to. Block only one and the other two quietly compensate, which is why doses keep climbing without much to show for it. Cilnidipine relaxes the arteries, telmisartan blocks the hormone, and chlorthalidone removes the retained fluid, each at a modest dose. Cilnidipine is the fourth-generation calcium blocker that acts on N-type as well as L-type channels, which is why it causes far less ankle swelling than amlodipine. Strip of 10 tablets, manufactured for Lupin by Archimedis Healthcare.
Key Features
- Cilnidipine 10 mg + Telmisartan IP 40 mg + Chlorthalidone IP 12.5 mg
- Three mechanisms at low doses rather than one drug at a high dose
- Cilnidipine – L/N-type blockade, so much less ankle swelling than amlodipine
- Chlorthalidone outlasts hydrochlorothiazide and has the stronger outcome record
- Strip of 10 tablets from Lupin Ltd.
Composition
- Cilnidipine – 10 mg
- Telmisartan IP – 40 mg
- Chlorthalidone IP – 12.5 mg
- Approved colours used. Excipients q.s. As printed on the pack
Uses & Benefits
- Lowers blood pressure that has not come down on one or two medicines
- Three different mechanisms at low doses instead of one drug pushed to a high dose
- Cilnidipine widens arteries with far less ankle swelling than amlodipine
- Chlorthalidone removes the salt and water retention that blunts the other two
- Telmisartan protects the kidneys and heart over the long term
- One tablet a day instead of three separate prescriptions
Directions for Use
- Swallow the tablet whole with water. Do not crush or chew it, as printed on the pack
- Take it once a day at the same time each morning – the diuretic works better early than at bedtime
- It can be taken with or without food, but keep the routine consistent
- Take it exactly as your doctor directs. The dose is decided by your readings, not by how you feel
- Do not stop on your own when your readings improve – that is the medicine working, not the problem going away
Precautions
- Do not take this if you are pregnant or planning a pregnancy. The pack carries a warning that it must be stopped as soon as pregnancy is discovered – telmisartan harms the developing baby in the second and third trimesters
- Your doctor will check potassium, sodium, uric acid, blood sugar and kidney function periodically. Chlorthalidone can lower potassium and sodium while raising uric acid and sugar; telmisartan pushes potassium the other way
- Tell your doctor if you have gout, diabetes, kidney or liver disease, or a low blood volume from vomiting or diarrhoea
- Avoid potassium supplements and salt substitutes unless your doctor approves them. Aliskiren is not to be combined with an ARB in diabetes
- Dizziness on standing is most likely with the first dose – get up slowly, and do not drive until you know how it affects you
- Report a persistent dry cough, swelling of the face or lips, unusual cramps or a very low urine output straight away
Storage
- Store below 30°C, protected from light and moisture, as printed on the strip
- Keep the tablets in the blister until you take them
- Keep out of reach of children
