Eplerite 25 from Cipla contains Eplerenone IP 25 mg in a film-coated tablet, manufactured by Synokem Pharmaceuticals at Haridwar. Eplerenone is a selective aldosterone antagonist, and its main use is a specific one: started in the days after a heart attack in patients whose heart muscle has been left weakened, where it has been shown to improve survival. The reason it helps is that a damaged heart triggers a flood of aldosterone, the hormone that makes the kidney retain salt and water. That overload raises blood pressure, loads the weakened pump with fluid, and over months drives the scarring that stiffens heart muscle further. Eplerenone blocks the receptor that the hormone acts through, interrupting the cycle. It is also used in chronic heart failure and in blood pressure that has resisted other treatment. Directions for Use

  • Take exactly as prescribed, usually once daily at the same time
  • Swallow the tablet whole with water; may be taken with or without food
  • Keep every blood test appointment. Potassium and kidney function are checked before starting, within the first week, and regularly after – this is not optional with this medicine
  • Do not take potassium supplements or potassium-based salt substitutes unless your doctor specifically tells you to
  • Do not stop or change the dose on your own, even if you feel well

Precautions

  • Raised blood potassium is the main risk, and it can be dangerous. Report muscle weakness, tingling, an irregular or slow heartbeat, or unusual tiredness without delay
  • Avoid potassium supplements, salt substitutes containing potassium, and potassium-sparing diuretics such as spironolactone or amiloride
  • Tell your doctor about kidney disease – eplerenone is not used when kidney function is significantly reduced
  • Do not take with ketoconazole, itraconazole, clarithromycin, ritonavir or nelfinavir – they block its breakdown and push potassium dangerously high
  • Tell your doctor about type 2 diabetes with kidney involvement, liver disease, or if you take ACE inhibitors, ARBs, NSAIDs or lithium
  • Dizziness, tiredness and diarrhoea can occur. Discuss pregnancy and breastfeeding with your doctor before use

Storage

  • Store protected from moisture, at a temperature not exceeding 30°C, as printed on the pack
  • Keep tablets in the original blister until use
  • Keep out of reach of children
