Barijak 4 mg Tablet from Cipla contains Baricitinib 4 mg as a film-coated tablet. Baricitinib is a JAK inhibitor – a targeted synthetic disease-modifying drug that works inside the cell rather than on it. Inflammatory signals such as the interleukins reach a joint by docking on the cell surface, but the message only becomes action once Janus kinase enzymes relay it to the nucleus. Baricitinib blocks JAK1 and JAK2, so the relay never happens and the inflammatory programme is never switched on. That is why it can calm rheumatoid arthritis that has not responded to methotrexate, and why it is taken as a daily tablet instead of an injected biologic. Pack of 14 tablets. Directions for Use

• May be taken with or without food, at the same time each day
• Do not stop or restart on your own – stopping suddenly can let the disease flare
• Keep every blood-test appointment your doctor books; this medicine is monitored, not simply dispensed

Precautions

• Baricitinib suppresses the immune system. Serious infections – including tuberculosis, shingles and hepatitis B reactivation – can occur. Your doctor will screen you for TB and hepatitis before you start
• Tell your doctor immediately about fever, cough, weight loss, night sweats or a painful blistering rash, and stop the tablet until you are assessed
• Regular blood tests are required – blood counts, liver enzymes and cholesterol are all monitored during treatment
• There is a raised risk of blood clots in the legs and lungs. Report leg swelling or pain, breathlessness or chest pain urgently
• JAK inhibitors carry warnings about major cardiac events and certain cancers, particularly in older smokers – your doctor weighs this before prescribing
• Not for use in pregnancy; effective contraception is needed during treatment and for a week after. Avoid breastfeeding
• Do not take live vaccines while on treatment. Tell your doctor about kidney or liver disease and every other medicine you take
