Poxet-30 from Sunrise Remedies is a Dapoxetine HCl 30 mg tablet for premature ejaculation, taken only when needed. The 30 mg strength is where treatment normally starts, and there is a reason not to skip past it. Dapoxetine’s characteristic problem is not lack of effect but orthostatic intolerance – a sudden fall in blood pressure on standing that can cause dizziness or fainting, most likely with the very first dose. That risk is dose-related, so the sensible approach is to establish tolerance at 30 mg before a doctor considers 60 mg. Taken with a full glass of water, and with alcohol avoided, most men tolerate it without trouble. What it does is straightforward: by raising serotonin activity briefly in the reflex pathway that controls ejaculation, it stretches the time to ejaculation severalfold for a few hours. Directions for Use

Precautions

  • Do not take this with, or within 14 days of, an SSRI, SNRI, MAOI, tricyclic antidepressant, lithium, tryptophan, St John’s wort or thioridazine. Combining serotonergic drugs risks serotonin syndrome
  • Fainting is the effect to know about. Dapoxetine can cause a sudden drop in blood pressure on standing, with dizziness or loss of consciousness – usually within the first three hours of the first dose. Drink a full glass of water with the tablet, and sit or lie down at the first sign of light-headedness
  • Do not drink alcohol with it. Alcohol adds to the dizziness and fainting risk
  • Not for men with significant heart disease, heart failure, an irregular heartbeat, a history of fainting or mania, moderate to severe liver disease, or under 18 / over 64
  • Do not drive or operate machinery after a dose until you know how it affects you. Nausea, headache, dizziness and diarrhoea are the common effects
  • Tell your doctor about every medicine you take – ketoconazole, ritonavir, clarithromycin and similar drugs raise dapoxetine levels sharply
