One practical point about chlorthalidone is worth knowing before you start: because it acts for so long, its effect builds over the first two to three weeks rather than arriving at once. Judging the combination by the reading after a few days is misleading – blood pressure usually keeps drifting down for a fortnight or more. Precautions

• Seek emergency care for swelling of the face, lips, tongue or throat

• Dizziness on standing is common early on, and worse if you are dehydrated
• Thiazide-type diuretics increase sun sensitivity – use sun protection
